Facilitators and barriers to seamless cross-sectoral handovers of adult somatic patients: Protocol for a scoping review

This scoping review aims to provide a comprehensive overview and categorization of barriers to and facilitators for achieving seamless cross-sectoral adult somatic patient handovers, related to patient involvement, information sharing and interpersonal relations, identified in the scientific literature.
