Description
The overall purpose of this action research project is to develop research-based models that support coherent healthcare for patients with multimorbidity across professions and sectors. An essential aim is also to prevent unnecessary re-admissions at hospitals.
The research project includes patients with multimorbidity, relatives, and clinicians.
