Abstract
TO THE EDITOR: We have read with interest the very timely review by Drs. Schwartz and McDermott (Schwartz TH, McDermott MW. The Simpson grade: abandon the scale but preserve the message. J Neurosurg. 2021;135[2]:488-495).1 We share the authors’ concern that the transparency and reproducibility of Simpson grading do not reach present standards.
Schwartz and McDermott describe different aspects of Simpson grading,2 but the major focus is Simpson grading as a predictor of recurrence. They list several limitations of the grading system in their Table 2. Subjective grading and limited reproducibility are undisputable
