Enriched power of case-only design in detecting gene-gene interaction using disease concordant twins

Abstrakt

Epistasis, or interactions among genes, have long been recognized to be fundamentally important to understanding both the structure and function of genetic pathways. Detecting gene-gene interaction has been a challenging issue in genetic epidemiology due to the complexity of the interactive genome that requires large sample sizes to ensure statistical power. We introduce a disease concordant twin case-only design that applies the powerful case-only design to disease concordant twins to address this issue. In this study, we examined the power advantage of the twin case-only design using computer simulations across a series of parameter schemes with different minor allele frequencies and relative risks. Results from the simulation study revealed a large sample size reduction by the twin case-only design. Sample size for dizygotic and monozygotic twin were roughly 1/2 and 1/4 of sample size in ordinary case-only design, and case-only design yields higher power compared to case-control design with equal number of cases. Our simulation results revealed high value of the innovative design by taking advantage of disease-concordant twins and the case-only design in detecting gene by gene interactions.
