3D lumbar spinal motion and low back pain

The overall issue of this project is to investigate the relationship between the lumbar spinal biomechanics, patient's pain and physiological effects of traditional conservative treatments.

There is a lack of fundamental knowledge of low back pain (LBP). How/why does the pain occur?  What is the origin of the pain? What mechanisms of action do the different kinds of conservative treatments have?

Kinematic factors such as velocity, acceleration is believed to play a role in relation to development of the LBP and development of degenerative changes.

The project is divided into tree parts.  Part 1 is a systematic critical literature review in order to determine the current level of evidence for measuring lumbar spinal motion. I will assess the various methods used to measure regional lumbar motion and, based on the review, propose an agenda for future research.  Part 2 is about development of a model to analyze dynamic non-invasive computerized regional motion data. In part 3 I will use the developed model on data already collected in randomized clinical trials, with blinded examiners on 301 CLBP divided in 3 groups with different conservative interventions, including 12 weeks supervised low-tech trunk exercises, Lumbar manipulation and a home exercise group. The motion data was collected at baseline and again after 3 months. Clinical data was collected after 5 and 11 weeks and follow-up after 1 year.  In addition we are planning a collection of spinal motion data on twins.

Some of the hypotheses: 3D CA-6000 data can be analyzed and expressed quantitatively as an expression for the low back motion biomechanics. Low back 3D motion pattern correlates with pain and clinical status. Various conservative treatments affect the low back in different ways and can be measured as a biomechanical difference between the interventions. The biomechanical movement pattern change in relation to the treatments and correlates with the improvement in the pain level. People without LBP have a different low back movement pattern from CLBP and can be measured with CA-6000.
